The Teenage and Young Adult Service in the East Midlands has created an aftercare pathway to ensure that your after treatment follow-up care is tailored to fit your specific needs.
At the end of treatment you will be transitioned into the TYA Survivorship Service for five years where you will be allocated a clinical nurse specialist who will oversee your care and long term follow up. Once you reach five years post treatment you will be transitioned into the self-managed follow up which is lifelong and monitors for any effects of the treatment for the rest of your life.
